responsibilities Task Build meaningful connections with Participants and their families, understanding and supporting their unique needs, goals, and daily activities. Assist with daily living tasks including meal preparation, medication administration, dressing, house hold duties, showering, and toileting as per individual plans.

Facilitate

Participants' engagement in activities, outings, education, or employment aligned with their individual plans, including transportation. Record shift notes, case updates, incidents, and medication administration using BoraCare systems, ensuring timely and accurate documentation. Uphold a clean, secure, and hygienic environment, supporting cultural and religious needs, and participating in evacuation procedures as needed.

Work collaboratively with colleagues and stakeholders, attending team meetings, core training sessions, and maintaining open communication. Follow protocols for incidents, near misses, and timesheets, providing feedback and reports to senior staff or management as required. Assist with planning, advocacy, and communication to ensure Participants’ personal goals and well-being are prioritized.

Key responsibilities Develop meaningful and professional relationships with Participants and their family, understanding their unique needs, goals, and daily living activities. Provide direct care (daily living and household activities) assistance to Participants in line with their individual plan, as required such as meal assistance, medication, dressing, household duties, showering ,and toileting.



Provide assistance with daily planning, advocacy, communication and transport as required.

Provide physical assistance as required including all aspects of manual handling, lifting, bending, stretching. Support engagement in activities, outings, education and/or employment related in line with individual plan. Ensure completion of appropriate documentation.

Relevant information on support provided is to be recorded in case notes and communication logs/handovers. Support and sustain the cultural and religious needs of Participants. Qualification &

experience Qualification &

experience Minimum Certificate III in Disability Services or in a relevant field, or higher. Maintaining all Industry and organizational related mandatory clearances and training requirements. At lest have 12 months or 24 months work experience in similar field.

Ongoing commitment to accurate record keeping confidentiality, duty of care. Effective written communication skills. Genuine interest in the wellbeing and inclusion of people with disability. Effective verbal communication skills and demonstrated flexibility in methods of communication, particularly active listening.

Demonstrated ability to problem solve and use initiative appropriately. Demonstrated capacity to collaborate as a part of a diverse team and work autonomously.
