Marathon Health Opportunities in New South Wales is seeking Care Coordinators or Aboriginal Health Workers to join our team in the Wagga Wagga and Gundagai/Tumut region. This role focuses on delivering person-centred care to clients with chronic diseases, working with GPs, nurses, and Aboriginal medical organisations to improve health outcomes.

You'll coordinate assessments, develop care plans, and connect individuals with allied services, while staying culturally secure and responsive to CALD
