This role focuses on delivering person-centred care to clients with chronic diseases, working with GPs, nurses, and Aboriginal medical organisations to improve health outcomes. You'll coordinate assessments, develop care plans, and connect individuals with allied services, while staying culturally protected and responsive to CALD
