Personal Care Worker / Personal Care Assistant

Victorian Transition Centre by VTC Health

Help people feel safe, supported and ready for their next step after hospital.

VTC Health is seeking compassionate and reliable Personal Care Workers / Personal Care Assistants to join our team at the Victorian Transition Centre in Preston.

If you take pride in providing thoughtful personal care, noticing when someone needs extra support and helping people maintain their independence, we would like to hear from you.

Experience working in a residential aged care facility is highly desirable.

About VTC Health

VTC Health’s purpose is to help people move safely from hospital into the community.

Our Preston centre is designed to provide short-term supported accommodation and coordinated care for older people, people with disability and adults recovering from illness or injury. Residents receive support while preparing to return home or move into their next care or accommodation setting.

For our care team, this means helping each person settle in, feel comfortable, maintain daily routines and work towards their individual goals.

Your role

You will provide personal care and practical daily support that respects each resident’s needs, preferences and choices.

Working closely with the Centre Manager, nursing staff and wider support team, you will follow individual care plans, communicate changes in residents’ needs and contribute to a safe, welcoming environment.

Your day-to-day responsibilities will include:

- Assisting with showering, dressing, grooming, toileting and continence care while protecting residents’ privacy and dignity.

- Supporting mobility, transfers and repositioning using the equipment and techniques specified in care plans and within your training.

- Assisting with meals, drinks and daily routines in line with each resident’s care needs.

- Encouraging residents to make choices, participate in daily activities and maintain their independence.

- Providing companionship and reassurance to residents adjusting to life after hospital.

- Recognising and promptly reporting changes in residents’ wellbeing, behaviour, mobility or support needs to the appropriate nurse or supervisor.

- Completing accurate care records and contributing to clear, reliable handovers between shifts.

- Following infection prevention and safe work procedures, reporting incidents and hazards, and helping maintain a clean and comfortable environment.

What you’ll bring

You are patient, dependable and attentive to the small things that make a difference to someone’s day. You communicate respectfully, work well with others and know when to ask for guidance.

We are looking for:

- A Certificate III in Individual Support, or an equivalent qualification in aged care or disability.

- Practical personal care skills and an understanding of safe manual handling and infection prevention.

- The ability to follow individual care plans and provide support that reflects residents’ preferences and cultural needs.

- Transparent communication skills and the ability to maintain accurate care documentation.

- A reliable approach to shifts, teamwork and continuity of care.

- Sound judgement when recognising concerns and seeking assistance.

Residential aged care experience is highly desirable. Experience supporting people with disability, complex support needs or people transitioning from hospital would also be valued.

Why join VTC Health?

- Work with a clear purpose: Help people through an important stage between hospital and their next home or care setting.

- Be part of a coordinated team: Work alongside nursing and support staff, with guidance from the Centre Manager.

- Bring your care experience to a varied role: Support residents with different backgrounds, needs and transition goals.

- Make a daily difference: Help residents feel heard, comfortable and confident while maintaining as much independence as possible.

How to apply

Please submit your CV and a short cover letter outlining your qualifications, relevant care experience and availability for rostered shifts.
