MyCareer Hub is recruiting on behalf of Care Services Australia (CSA) for an experienced Care Manager (Aged Care) to support clients under the Home Care Package (HCP) program.

This role is responsible for managing and coordinating services for a portfolio of 50+ aged care clients, ensuring all care plans, services, and documentation align with the Aged Care Quality Standards and organisational requirements. The successful candidate will work closely with clients, families, support workers, allied health providers, and external partners to ensure high-quality, person-centred care.

Key Responsibilities

Client Care Management

- Manage a caseload of 50+ Home Care Package clients
- Conduct client assessments and develop person-centred care plans
- Review and update care plans based on client needs and goals
- Monitor client wellbeing and escalate risks or changes when required

Service Coordination

- Coordinate services including support workers, allied health providers, and suppliers
- Liaise with clients, families, GPs, hospitals, and community organisations
- Manage service requests and ensure timely delivery of supports
- Review service delivery to ensure client satisfaction and outcomes

Compliance & Documentation

- Maintain accurate progress notes, care plans, and client records
- Ensure services align with Aged Care Quality Standards
- Maintain compliance documentation and service records




- Support internal audits and quality improvement activities

Support Worker Coordination

- Provide guidance and support to support workers delivering services
- Ensure shifts are covered and services are delivered as scheduled
- Address service-related enquiries and resolve issues when required

Requirements

- Experience in Home Care Packages, aged care coordination, or case management
- Robust understanding of Aged Care Quality Standards
- Excellent communication, organisation, and time management skills
- Ability to manage a large client portfolio and coordinate multiple services
- Experience using care management systems (e.g. Visual Care, Procura, Lumary or similar) is desirable

Desirable

- Nursing qualification (EN/RN) or relevant health background
- Experience working within community or home care services

Job Type: Full-time

Pay: $75,000.00 – $85,000.00 per year
