What you'll do:

- Conduct clinical assessments and develop personalised care plans.

- Deliver nursing care, including medication management and wound care.

- Monitor clients' health and wellbeing.

- Guide and support care staff.

- Work closely with clients, families and healthcare professionals.

- Maintain clinical documentation and compliance standards.

- Current AHPRA registration as a Registered Nurse.

- Bachelor of Nursing or equivalent qualification.

- Minimum 2–3 years of experience as a Registered Nurse.

- Knowledge of Support At Home Program and consumer-directed care.

- Understanding of aged care legislation and clinical compliance requirements.

- Current National Police Check.





- Current First Aid and CPR certification.

- Valid Australian driver's licence and access to a reliable, insured vehicle.

- Professional Indemnity Insurance.

- Experience in clinical assessments, care planning and staff supervision.

- Passion for delivering high-quality, person-centred care.

- Excellent communication and interpersonal skills.

- Strong clinical assessment and decision-making abilities.

- Ability to work independently and as part of a team.

- Strong leadership, mentoring and problem-solving skills.

- Excellent time management and organisational abilities.
