Centre Manager – Preston
Victorian Transition Centre by VTC Health
Lead our team and oversee the daily running of our Preston centre.
VTC Health is seeking an experienced and practical Centre Manager to take responsibility for the day-to-day operations of our Victorian Transition Centre in Preston.
We are looking for an approachable leader who enjoys being involved in the daily running of a service—supporting staff, coordinating care, resolving issues and ensuring residents receive safe, consistent and respectful support.
About VTC Health
VTC Health’s purpose is to help people move safely from hospital into the community.
Our Preston centre is designed to provide short-term supported accommodation and coordinated transition support for adults who are ready to leave hospital but need assistance before returning home or moving into longer-term care or accommodation.
The service supports pathways for people with disability, people recovering from illness or injury, and older people requiring additional care and support.
About the role
As Centre Manager, you will oversee the daily operation of the Preston site, ensuring the centre is appropriately staffed, services are coordinated and residents’ needs are met.
You will lead the onsite team, maintain clear routines and expectations, and be the main point of contact for daily operational matters. Working closely with VTC Health’s leadership and clinical team, you will ensure staff have the guidance, resources and support they need to deliver quality services.
Key responsibilities
- Daily site management: Oversee the running of the centre, manage competing priorities and resolve operational issues promptly.
- Staff leadership and supervision: Provide clear direction, support and feedback to staff, oversee induction and training,
and maintain a positive and accountable team culture.
- Rostering and workforce coverage: Manage rosters, staff allocation, leave and unexpected absences to maintain safe and reliable service delivery.
- Resident care and experience: Ensure daily support reflects each resident’s care plan, preferences and goals, and respond to concerns raised by residents, families and carers.
- Admissions and transitions: Coordinate the practical arrangements for arrivals, hospital handovers and departures, ensuring staff have the information and resources required.
- Quality and safety: Monitor documentation, handovers and adherence to policies. Oversee incident and complaint processes, and work with clinical staff on medication management systems, care concerns and appropriate escalation.
- Facilities and daily services: Oversee the cleanliness, presentation, maintenance and safety of the site, including coordination of meals, housekeeping, supplies and external service providers.
- Communication and coordination: Maintain effective daily communication with families, hospital teams, support coordinators, allied health professionals and other providers involved in residents’ care.
- Resources and reporting: Manage staffing and site expenditure within approved budgets, maintain accurate records and keep leadership informed about service performance, risks and operational needs.
About you
You are organised, dependable and confident leading a team in a busy care setting.
You take ownership of daily operations, address problems early and balance the needs of residents, staff and the service.
Experience managing a residential aged care facility is highly desirable.
You will bring:
- Experience managing a centre, facility, residential service or operational team within disability, aged care, community health or a related setting.
- Demonstrated ability to manage staff, rosters and daily service delivery.
- A practical understanding of supporting people with complex care or support needs.
- Knowledge of safeguarding, incident management, documentation and quality improvement.
- Robust communication skills and the ability to build respectful relationships with residents, families, staff and external professionals.
- Sound judgement, including recognising when to seek clinical advice or escalate an issue.
- Relevant qualifications in disability, aged care, nursing, allied health, health services management or a related field, or substantial equivalent management experience.
Experience overseeing a service that provides 24-hour care or support would also be highly regarded.
Why join VTC Health?
- Take ownership of the daily operation of our Preston centre.
- Lead a team delivering meaningful support to people transitioning from hospital.
- Work closely with executive leadership and access clinical governance, operational and finance support.
- Help create a welcoming environment where residents feel respected and staff feel supported.
