Job Title: Support Coordinator

Position Overview:
We are seeking a dedicated and experienced Support Coordinator to assist NDIS participants in achieving their goals by connecting them to appropriate services and supports. This role places a solid emphasis on providing support to participants with psychosocial disabilities, leveraging your experience in psychological recovery coaching to empower individuals on their recovery journey.

Key Responsibilities:
Collaborate with NDIS participants to identify goals and develop individualized support plans.
Coordinate and connect participants with suitable services, providers, and community resources.
Actively support participants experiencing psychosocial challenges by promoting self-awareness, empowerment, and recovery-oriented approaches.
Monitor and review participant progress, ensuring services align with their needs and NDIS plan goals.
Advocate for participants, ensuring their voices are heard and choices respected.
Provide coaching to participants, supporting them to build skills, resilience, and independence as part of their psychological recovery journey.
Maintain accurate, up-to-date documentation and reports in compliance with NDIS guidelines.




Assist participants to navigate and resolve any barriers impacting their plan implementation.

Key Requirements:
Experience as a Support Coordinator with robust understanding of NDIS frameworks and practices.
Demonstrated experience or qualifications in psychosocial recovery coaching or mental health support.
Excellent communication and interpersonal skills with the ability to build trusting relationships.
Strong organizational and problem-solving abilities to manage participant plans effectively.
Knowledge of recovery-oriented principles and person-centered approaches.
Relevant qualifications in Social Work, Psychology, Mental Health, or a related field (desirable).

Job Types: Full-time, Subcontract, Casual

Pay: $45.00 – $60.00 per hour

Expected hours: 15.0 – 30.0 per week
