Grampians Health is seeking a Memory Support Nurse for Stawell Community Services on a fixed term, part time basis (0.4 FTE) until July 2027. The role focuses on dementia care, assessment and care planning, working with consumers, families and the broader health team to deliver person-centred, quality care.

You will conduct memory assessments, develop care plans, provide dementia education and coordinate referrals to ensure integrated community services across the Grampians region.
