Job Responsibilities:
Providing in-home care and support to clients, including assistance with personal care, meal preparation, and household tasks
Assisting clients with mobility and transportation to appointments and outings
Building rapport and developing relationships with clients to ensure their comfort and well-being
Communicating regularly with clients and their families to provide updates and address any concerns




Maintaining accurate records of care provided and reporting any changes in client's condition to the supervising nurse or case manager
Job Requirements:
Must have a full Australian driver's license and access to a reliable vehicle for transportation
Valid police check and working with children check
Previous experience in home and community care or a related field is preferred
A compassionate and caring attitude towards clients and a commitment to providing high-quality care
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Flexibility to work a variety of shifts, including evenings and weekends
