Dream Maker Community Services is currently recruiting for a Residential Key Worker to join our team at Leumeah Residence on a permanent, full time basis.

The key responsibilities includes the following but not limited to:

- Coordinate and oversee the delivery of care services to clients (including children) in residential settings.

- Liaise with clients, families, care workers, and external stakeholders to ensure individual care needs are met.

- Develop, monitor, and review individualised care plans.

- Oversee the day-to-day operations of residential properties, ensuring services align with NDIS Quality and Safeguarding Standards

- Ensure compliance with organisational policies, funding guidelines, and aged care standards.

- Provide ongoing support, supervision, and guidance to support staff.

- Maintain accurate and confidential client records and reporting systems.

- Respond promptly to incidents, complaints, and feedback in a professional and empathetic manner.

- Schedule and lead staff meetings, ensuring alignment across service delivery and organisational goals

- Participate in quality improvement initiatives and contribute to service development.





The successful candidate needs to possess the following experience and skills:

Qualifications & Experience

- At least a Diploma or higher in Community Services, Aged Care, Disability, or a related field.

- Minimum 1 years experience in a residential care coordination or similar client-focused role.

- Sound knowledge of aged care or disability legislation, standards, and person-centred practices.

- Extensive knowledge of NDIS Practice Standards, Quality and Safeguarding Framework, and NDIS Code of Conduct

- Current Police Check, Working with Children Check (WWCC), and NDIS Worker Screening Check

- Valid Australian Driver’s Licence and Current Valid Full Time Working Visa

- Strong interpersonal and communication skills with the ability to build rapport with diverse stakeholders.

- Excellent organisational and time management abilities.

- A proactive, empathetic, and solutions-focused approach to service delivery.

- Proficiency in Microsoft Office and client management systems.
