Direct Care Australia on the Gold Coast is seeking an experienced Care Partner to join the Community Aged Care team. You will manage a caseload, conduct assessments, develop care plans and coordinate services to support older people to live safely and independently at home.
The role requires robust documentation, professional judgement, and the ability to travel across the Gold Coast, Brisbane and Sunshine Coast. A company vehicle is provided; this is a full-time, ongoing position.
