Journey Support Coordinator
Part-time | Cooktown & South East Cape York | Start immediately
1. Are you a good listener?
2. Are you kind, reliable and confidential?
3. Do you want a supportive and adaptable workplace?
Cooktown District Community Centre is looking for the right person to join our Journey Universal Aftercare team.
For this role, who you are matters.
We are looking for someone who understands our local communities, knows how to connect with people and can walk alongside someone going through a difficult time, providing support in a way that builds confidence, independence and choice.
Journey provides practical, emotional and social support to people following a suicidal crisis or suicide attempt. It is a non-clinical support role. The Journey worker listens, stays connected, helps with practical needs and supports people to connect with family, community and other services.
The clinical side of a person's care remains with Torres and Cape Hospital and Health Service or another appropriately qualified clinical service.
The person we are looking for
You might be right for this role if you: · are kind, calm and a good listener, and can build trusting, respectful relationships without judgement
· can support people in a way that builds confidence, independence and choice, particularly when they are going through difficult times
· are reliable and dependable, keep appointments and follow through on what you say you will do
· have strong local cultural knowledge and community connections, understand how mental health concerns can affect individuals and families, and know, or are willing to learn about, support services across South East Cape York
Journey's approach is based on safety, choice, trust, compassion, connection and collaboration,
with support grounded in culture, community and the participant's own choices.
You don't need to know everything on day one
For us, who you are is just as important as your previous work experience. Experience in community services, mental health, wellbeing, health, outreach or similar work would be valuable, but strong community knowledge, transferable skills and a willingness to learn are just as important.
Aboriginal and Torres Strait Islander people are strongly encouraged to apply. This is not an identified position and all applications will be assessed on merit.
About the position
· Part-time: 22.8 to 30.4 hours per week. · Fixed-term contract to 30 June 2027, with a view to extension subject to future funding. Immediate start.
· Community and outreach-based work across the local region
· Training and professional supervision provided, supportive team environment within a supportive organisation.
Work that fits around life
At CDCC, we genuinely understand that our staff have families, children and commitments outside work. Working parents are welcome.
We are happy to talk about an arrangement that works for the service, the successful candidate and their family responsibilities. The advertised hours can be arranged across five days or fewer.
What will you actually do?
A lot of this role is simply about being there and following through.
You will
- listen, stay connected and provide practical and emotional support based on what matters to each person
- help people connect with family, community, cultural, health and other services, including helping them stay connected with important appointments and supports
- work respectfully with families, community, Torres and Cape Hospital and Health Service and other local services to support the person’s wellbeing and choices
- work safely and responsibly, keeping clear and confidential records and seeking help when there are concerns about someone’s safety
The Journey model specifically emphasises listening without judgement, practical support, community connection and helping participants work towards manageable steps that matter to them.
Most importantly... We're looking for someone people can rely on.
· Someone who listens. · Someone who does what they say they will do.
· Someone who supports people in a way that builds confidence, independence and choice.
· Someone who knows our communities and treats people with dignity and respect.
If that sounds like you, we'd really like to hear from you.
How to apply
You can: · send us your resume and a short letter telling us why you think you would be a good person for this role; or
· if writing an application isn't your thing, give us a call and come in for an informal chat.
You don't need to write a complicated application. We want to hear about you, your connection with community, your experience and what you could bring to Journey.
For a confidential discussion, contact Cooktown District Community Centre on (07) 4082 1400 4082 1400) and ask to speak with Tracey or Kath.
