The Oaks Aged Care Facility is looking for a warm, creative and organised Lifestyle Coordinator to join our team in Yahl, South Australia, just 10 minutes from beautiful Mount Gambier, with its world-famous Blue Lake, sinkholes and caves. This full time role is available to start as soon as possible.

About the role

You will plan, coordinate and evaluate lifestyle and leisure programs that support our residents’ wellbeing, independence and quality of life. Reporting to the Facility Manager, you will lead a program that is meaningful, person-centred and shaped around what each resident enjoys and values. You will also support compliance with the Aged Care Quality Standards.

What you’ll be doing

- Planning and running group activities, including exercise, cognitive, social, cultural, music and creative programs

- Providing one-to-one engagement programs and support strategies

- Completing lifestyle assessments, and developing and reviewing individual lifestyle care plans

- Keeping accurate and timely documentation, including attendance records and progress notes

- Coordinating outings, visitors, entertainment and community links

- Leading and mentoring Lifestyle Assistants, volunteers and students

- Taking part in multidisciplinary team meetings, audits, quality improvement and accreditation activities

- Promoting a safe, engaging and supportive workplace for residents, staff and visitors

About you

- A genuine commitment to contemporary, person-centred aged care

- Experience as a Lifestyle Coordinator, Lifestyle Assistant supervisor or similar role in aged care

- Empathy, patience and an understanding of the needs of older people and their families

- Knowledge of dementia and appropriate support strategies





- Ability to design programs that address residents’ physical, psychological, emotional, social, cultural, spiritual and creative needs

- Knowledge of aged care documentation, accreditation requirements and funding processes

- Leadership skills, with excellent organisational, communication and teamwork abilities

- Computer literacy, including Microsoft Office.

Experience with aged care software is desirable

- Qualification: Certificate IV in Leisure and Health (essential or working towards)
