Key Responsibilities

- Assisting the Care Coordinator with clinical documentation, care planning, and leadership of the care team
- Leading and supporting new consumer admissions and care planning processes
- Communicating effectively with families, allied health professionals, and other care partners
- Fostering a welcoming, inclusive environment for residents and staff
- Actively participating in quality improvement and risk management activities
