About the role

As a Specialist Support Coordinator, you will work with participants who have complex needs, helping them overcome barriers, navigate challenging situations, and connect with the services and supports they need to achieve their goals.

Key responsibilities

- Providing Specialist Support Coordination to NDIS participants with complex needs.
- Developing and implementing individualised support plans.
- Coordinating multidisciplinary and complex support arrangements.
- Building strong relationships with participants, families, service providers, and key stakeholders.
- Identifying and addressing service gaps, barriers, and risks.
- Supporting participants through crisis situations and undertaking risk assessments where required.
- Monitoring NDIS funding, budgets, and service implementation.
- Maintaining accurate case notes, reports, and participant records.
- Ensuring compliance with NDIS Practice Standards, Quality and Safeguarding requirements.
- Reviewing participant progress and adjusting support strategies as needs change.

About you

- Strong interpersonal and relationship-building skills.
- Excellent verbal and written communication skills.
- Experience in case management, support coordination, or complex care planning.
- Knowledge of the NDIS and person-centred approaches.
- Experience supporting people with disability, psychosocial disability,



or complex support needs.
- The ability to work independently while collaborating effectively with multidisciplinary teams.
- Strong organisational and time management skills.
- A relevant tertiary qualification in Social Work, Social Science, Psychology, Community Services, Human Services, or a related field is required.
- Current driver's licence and relevant clearances.
