Australian Unity is seeking a Care Partner to guide customers through their care journey with compassion.
You will work with families, clinical teams, and external providers to develop holistic health plans and coordinate services, enabling safe, person-centred care at home or a location of choice.
This permanent full time role offers flexible hours (8:30am–4:30pm or 9:00am–5:00pm, Mon–Fri) and a hybrid work model with 2 days from home, 3 days in the Clayton office.
