Job Description
KEY RESPONSIBILITIES:
Client Intake:
Process referrals, assist with assessments, support plan documentation, meal selections and client enquiries.
Service Coordination:
Coordinate service schedules, meal deliveries, contractors, rosters and vehicle maintenance.
Administration:
Manage meal orders, invoices, payments, client records, data entry and reporting.




Communication:
Liaise with clients, families, care workers, contractors and meal vendors.
Compliance & Quality:
Maintain CHSP compliance, support audits and contribute to service improvements.
Community Engagement:
Support outreach, client surveys, marketing activities and community events.
General Support:
Assist colleagues and undertake other duties as required.
Desired Skills and Experience
QUALIFICATIONS & EXPERIENCE:
Relevant qualification in Community Services, Community Development, Aged Care or a similar discipline.
Solid customer service experience and experience working with clients from diverse backgrounds.
Knowledge or experience in CHSP, HCP, NDIS or other community services.
KNOWLEDGE:
Food safety, handling regulations and dietary needs of older people.
Aged Care Quality Standards.
Quality assurance and occupational health and safety, particularly for at-risk individuals.
