Hesse Rural Health in Victoria seeks a Credentialed Aged Care On-Site Pharmacist (ACOP) to provide expert medication management within a residential aged care facility. You will collaborate with GPs, in office nurses and care staff to optimize pharmacotherapy and resident outcomes.
This 2-day-per-week contractor role requires an ABN and ACOP credentialing, with a focus on safety, quality use of medicines and team-based care across rural community facilities.
