The role: You will be required to assist individuals living in ACH Group's North - Eastern residential care homes to live a good life characterised by optimum independence and decision-making about their care needs.What you'll do:Support the older person to maintain and/or improve physical, cognitive, emotional and social health and enhance their wellbeingFollow and contribute to the resident's care planAssist the residents of each care home to live a good lifeWork with a multi-disciplinary team to support the wellbeing of residentsUndertake activities of daily living, domestic tasks and activity supportMaintain accurate documentationAbout You!
