HV Care is seeking a Care Partner to join our hybrid team servicing Hervey Bay and Maryborough. The role focuses on coordinating aged care services, developing care plans and supporting clients and families within the Wide Bay region.

You will work across Support at Home, CHSP and allied health services, building solid relationships with clients, families and healthcare professionals while ensuring high-quality, compliant care. Hybrid work arrangement is offered to support flexibility.
