My Flex Health in Perth is seeking an experienced Care Partner/Coordinator to manage a caseload of Support at Home across Perth. This non-clinical role focuses on coordinating person-centred services that promote clients' independence, wellbeing, and achievement of goals within the community.

You will be based at our Perth office, with travel across North of the River to support clients, families, and stakeholders.
