SA Health in Port Broughton is seeking an Enrolled Nurse - Transition Care Program (TCP) Coordinator to support person-centred care and coordinate admissions for the TCP service. You will assess suitability, set goals, and work with families and the care team to plan safe transitions home.

You will collaborate with the Nursing Unit Manager to plan staffing resources and deliver TCP; flexible, part time arrangements may apply, with salary packaging options and superannuation.
