Chronic Care Australia is building a new value‑based oncology model in Perth that puts them at the centre, and we’re looking for a Physiotherapist who wants to be part of the collaborative allied health delivery. In this role you’ll triage new clients and manage their care through our value‑based model. You’ll work closely with our exercise physiologists and with each client’s oncologist to make sure every person gets the right care at the right time. The role at a glance

-
Experience:
4+ years as a registered Physiotherapist, with a real interest in oncology
-
Location:
Cottesloe clinic, 525 Stirling Hwy, Cottesloe WA, and our online virtual care clinic About Chronic Care Australia Chronic Care Australia (CCA) is leading reform in primary allied healthcare. We are a globally awarded clinic that designs and delivers value-based exercise medicine models of care, where success is measured by the outcomes patients actually experience. From our clinic in Cottesloe and our online virtual care clinic, we help people live well with chronic and complex conditions. Now we’re taking our next big step. CCA is the first provider in Western Australia to work with FITMED, and we are launching a dedicated exercise oncology service built on our value-based model. The service brings together exercise physiologists,
physiotherapists and specialist dietitians, FITMED’s new clinical platform and the wider cancer care team around each patient. Our goal is simple: make expert exercise care a standard part of the cancer journey for everyone who can benefit. For the right clinician, this is a rare chance to be there from day one, helping shape a service that could change how cancer care is delivered in Western Australia. Why this role is different
- Be at the front door of care. You’ll triage clients as they enter the service and manage their care through the value-based model. Working with exercise physiologists and oncologists, you’ll shape each person’s pathway from first contact to discharge.
- Build something, don’t just join something. You’ll be one of the first clinicians in a new value-based care model, with genuine input into how the service, programs and referral pathways take shape.
- Your outcomes count. Value-based care is about results that matter to patients. You’ll measure, track and show the difference rehabilitation makes, and see that evidence shape the service.
- Help write best practice. You’ll be part of real‑world evidence and research that will help develop a best‑practice clinical pathway for exercise medicine in value-based healthcare. Working alongside exercise physiologists and specialist dietitians, this is a rare opportunity to make a significant impact on your career.
- Be first in WA.
CCA is the first provider in Western Australia to work with FITMED. You’ll be trained in its new clinical platform, one part of this model, to deliver and track care across our Cottesloe clinic and virtual care clinic.
- Work across disciplines. You’ll work side by side with exercise physiologists and specialist dietitians, combining hands‑on physiotherapy expertise with exercise medicine and nutrition for better patient outcomes.
- Become an oncology rehabilitation expert. This is a real pathway to build specialist-level knowledge in one of the fastest-growing areas of physiotherapy. What you’ll do
- Triage new referrals, assessing each client’s needs, risks and goals to decide the right pathway and level of care within the value-based model
- Manage each client’s care across the pathway, coordinating with exercise physiologists on exercise programs and with oncologists on treatment plans, progress and any changes in medical status
- Assess and treat people at every stage of cancer care, from prehabilitation through active treatment, recovery and survivorship
- Design individualised rehabilitation and exercise programs targeting strength, mobility, function, fatigue and return to everyday life
- Manage common treatment effects such as post‑surgical shoulder and chest wall restriction, chemotherapy‑induced peripheral neuropathy, balance issues, lymphoedema risk, reduced bone health and pelvic floor or continence changes (within your scope and training)
- Own your caseload, from first assessment and goal setting through progress reviews, outcome measurement and discharge
- Run individual and group sessions in clinic and via telehealth
- Use sharp clinical reasoning and safety screening to know when to push, modify, pause or elevate
- Use FITM
