Senior Helpers Northern Beaches and Suburbs is seeking a Clinical Care Coordinator / Care Partner / Case Manager to support older Australians in their homes. This part time role based at Dee Why Office focuses on assessing clients, developing care plans, coordinating services and maintaining documentation. You will liaise with nurses and admin, manage a portfolio, and deliver compassionate, client-centred care.
