We are seeking a compassionate, reliable and experienced Female Disability Support Worker to support an NDIS participant in the Ryde area. The successful applicant will have experience supporting people living with dementia and a genuine passion for helping participants remain secure, engaged and connected with their community.

About the Role

This role involves providing community access and daily living support to a participant with dementia. The successful candidate will assist the participant to access the community, attend appointments and engage in meaningful activities while promoting safety, independence and wellbeing.

Community support shifts are required on:

- Tuesday
- Thursday
- Saturday
- Sunday

Applicants must be available for the above days and able to provide reliable, consistent support.

Key Responsibilities

- Provide person-centred support to a participant living with dementia.
- Assist with community access, social activities and recreational outings.
- Support the participant to attend appointments and community events.
- Provide transport using your own vehicle, as required.
- Promote independence, dignity, choice and safety at all times.
- Support the participant with daily living activities where required.
- Follow individual support plans and relevant safety procedures.
- Monitor and report changes in the participant’s health, behaviour or support needs.




- Maintain accurate shift notes and communicate effectively with the care team.
- Build a positive, respectful and trusting relationship with the participant.

Essential Requirements

- Previous experience supporting people living with dementia.
- Experience in disability support, aged care or community care.
- Female Support Worker – required for this participant’s support needs.
- Current NSW driver’s licence.
- Own reliable and insured vehicle suitable for community access.
- Current NDIS Worker Screening Check.
- Current First Aid and CPR certification.
- Strong communication, reliability and time-management skills.
- A compassionate, patient and person-centred approach.
- Right to work in Australia.

Desirable

- Experience supporting participants with complex or changing needs.
- Knowledge of dementia-informed care and strategies for supporting memory loss, confusion and changing behaviours.
- Experience with community participation and transport support.
- Relevant Certificate III or IV in Individual Support, Disability, Ageing Support or equivalent.
