About the role
Health U Australia is a registered NDIS provider delivering Supported Independent Living across three houses, community supports, and Support Coordination. We're a small, hands-on team and we're hiring an experienced professional for a genuine hybrid role: half support coordination, half quality and compliance.
This is not a support coordination job with some admin attached. Compliance is half the role, with protected time and a caseload sized to make that real. We're paying above a standard SC rate because we're asking for two skill sets, and we expect to find someone who has both.
We're not looking for someone to keep the paperwork tidy. We want someone who notices what could be done better and does it. If you've rewritten a form because the old one didn't capture what mattered, or set up a system that saved your team hours a week, that's exactly what this role rewards.
Our support coordination is kept separate from our other services, so your caseload will not include participants who receive SIL or community supports from Health U. Your compliance work, however, covers our SIL houses and community programs directly.
Key responsibilities
Support Coordination
- Manage an independent caseload.
- Manage intake for new participants, including documentation, consent and risk assessments
- Assist participants with NDIS applications, plan reviews and reassessments, and prepare the reports and evidence to support them
- Implement plans: identify, connect and coordinate suitable providers in line with each participant's goals
- Advocate for participants and ensure their rights and choices are upheld within the NDIS system
- Educate participants, families and support networks on the NDIS so they can navigate it with confidence
- Collaborate with allied health, community organisations and government agencies
Quality and Compliance
- Own the quality of documentation across our SIL houses and community supports: support plans, progress notes,
risk assessments and incident records
- Manage incident reporting, including reportable incidents to the NDIS Commission
- Keep Health U aligned with the NDIS Practice Standards and audit-ready, including restrictive practice obligations
- Audit staff records and coach support workers on documentation standards
- Identify gaps in how we work and lead improvements to policies, procedures and systems
Qualifications and skills
Essential
- 3+ years' experience as an NDIS Support Coordinator (Level 2 or 3), with a robust understanding of NDIS processes, pricing and legislation
- Hands-on NDIS compliance experience: audit preparation, incident management or Practice Standards implementation
- Excellent written English and report writing. The NDIA and the Commission read what you write
- A track record of improving something, not just maintaining it
- Able to work independently in a small, supportive team and manage a varied workload
- NDIS Worker Screening Check, Working with Children Check, driver's licence and access to a vehicle
Highly regarded
- Bachelor's degree or Diploma in Social Work, Disability, Community Services, Allied Health, or Quality/Governance
- SIL experience, or experience with participants with complex needs or regulated restrictive practices
- Experience with CRM or care management systems
Why Health U
- A real seat at the table: direct access to the director, team players and a say in how we document and deliver
- Variety: three SIL houses, community programs and an independent coordination caseload, so no two weeks look the same
- Your improvements get adopted. We're small enough that good ideas don't sit in a queue
- Professional development support
