Central Highlands Rural Health (CHRH) is currently seeking applications for a Program Assistant to join our Social Support Team based at Clunes Health on a permanent part-time basis.

About the Role

As a Program Assistant, you will play a valued role in supporting older people, including those living with dementia, to remain socially connected and engaged within their community.

The Social Support Program aims to enhance the wellbeing, independence, and quality of life of frail older people through structured, community-based group activities. These activities encourage social inclusion, meaningful participation, and the maintenance of independent living skills in a supportive and welcoming workplace.

The program is primarily funded through the Commonwealth Home Support Programme (CHSP).

Key Selection Criteria

To be successful in this role, you will demonstrate:

- Certificate IV in Ageing Support, Certificate III in Individual Support, or an equivalent qualification.
- Experience developing and delivering recreational or social activities for older people, people with disabilities, and/or those living with dementia.
- A genuine passion for supporting older people, particularly individuals living with dementia.
- A commitment to person-centred care and practice.
- Excellent communication and interpersonal skills.
- The ability to foster teamwork and contribute positively to a collaborative environment.
- Sound computer literacy skills
