Queensland Health Brisbane - North seeks a Clinical Nurse Discharge Planner for the Patient Access and Clinical Effectiveness (PACE) team. You will coordinate complex discharges and ensure secure transitions across acute, subacute, community and primary care services.

You will guide patients and families on community services, manage referrals, and support those with complex needs while contributing to readmission reduction and continuous improvement in care pathways.
