permanent Case Worker based in Rockhampton
with Lifestyle Solutions, you'll play a vital role in supporting children and young people in Out-of-Home Care to feel safe, connected, and empowered to thrive.
Through therapeutic case management, frontline coaching, and collaborative care planning, you'll help create meaningful and lasting outcomes for young people and their support networks.

What You'll Bring
Cert IV or Diploma in Community Services, Youth Work, Social Work or similar (or currently working towards)
Proven case management and strong stakeholder engagement skills
Demonstrated leadership skills, with the ability to lead, mentor and develop a team
Strong knowledge of trauma-informed and therapeutic care practices
Ability to oversee service delivery, promote consistent practice and drive positive outcomes for young people
Strong documentation, organisational and compliance capabilities
Current driver's licence and required probity clearances

What You'll Be Doing
Lead and review case plans to achieve therapeutic, cultural, and individual goals
Coach and support frontline staff to embed therapeutic strategies in everyday care
Engage with families, carers, and external agencies to ensure aligned and positive outcomes
Maintain accurate documentation and ensure compliance with relevant standards and legislation
Promote secure, nurturing, and culturally respectful environments for children and young people
Support the delivery of trauma-informed, evidence-based care practices across services
