The exciting opportunity:

- Manage your own caseload of Support at Home clients, developing strong relationships with clients and their families
- Conduct assessments, develop and review individualised care plans, and coordinate services as client needs change
- Monitor client budgets and funding arrangements to ensure services remain appropriate and within available funding
- Work collaboratively with support workers, healthcare professionals and the administration team to coordinate quality care
- Identify changes in client needs, respond to concerns and arrange appropriate support or clinical input where required
- Maintain accurate client documentation and contribute to quality and continuous improvement
- Work from one of seven office hubs across Perth Metro, depending on your preferred location

About you:

- Demonstrated experience working with Support at Home (SAH) clients is essential
- Experience conducting client assessments, developing care plans, coordinating services and managing individual client budgets
- Good understanding of aged care legislation, quality standards and contemporary home care practices
- Strong organisational and relationship-building skills, with the ability to manage competing priorities
- A warm, practical and client-focused approach, with a genuine commitment to supporting older Australians
- Comfortable working independently while contributing positively to a collaborative team
- Clinical qualifications are welcomed but are not essential
