The Care Partner - Support at Home will oversee and assess the services provided to clients, monitoring outcomes against each individual’s service plan, with an aim to optimize a person’s quality of life and achievement of goals.

Located at the Riverland Community Services office in Berri, this is a full-time permanent position. In this role, it is expected you will demonstrate personal responsibility, effective communication, and adaptability while consistently delivering high-quality service aligned with Resthaven’s values. This includes transparent and respectful communication, active listening, and providing timely updates.

Selection Criteria

Essential

- Demonstrated Program Coordination, Team Leader, case management experience
- An empathy and understanding of issues for older people, their families, and carers
- Knowledge and experience of quality systems for evidence-based care




- A commitment to supporting older people to remain at home in the community
- Proven ability to work in a holistic, person centred model of care
- An ability to work with individuals’ to identify their needs, develop personal goals and develop responsive packages of services
- Demonstrated computer literacy
- A current SA driver’s license

Desirable

. The following qualifications are highly recommended:

- Diploma of Nursing
- Certificate III in individual Support (Ageing) or Health Services Assistance
- Certificate IV in Aged Care, Disability or Community Services
- Diploma of Community Services (Case Management)
- Diploma of Ageing Studies and services
