Disability United Pty Ltd in Narre Warren South, Victoria, is seeking a dedicated Community Worker to assess participant needs, create personalised support plans and promote independence. You will facilitate programs, document progress, and coordinate with families and external providers to ensure seamless care.

The role requires trauma-informed practice, crisis de-escalation, and a person-centred approach within NDIS standards. Salary is AUD 80,000 per annum.
