Clinical Assessments Registered Nurse CareSupport2U is seeking multiple Registered Nurses across NSW to complete clinical nursing assessments in clients' homes and community settings.
Opportunities are available in metropolitan, regional, and rural areas. Nurses will generally work within their local area and have flexibility to manage their availability and workload.
You will conduct comprehensive clinical assessments, identify risks and support needs, and complete clinical reports and recommendations to support client care.
Training, education, assessment tools, templates, and ongoing clinical support will be provided.
Key Responsibilities
- Conduct comprehensive clinical nursing assessments
- Assess health status, functional capacity, risks, and care needs
- Complete clinical reports and recommendations
- Identify changes in health and escalate concerns when required
- Liaise with clients, families, providers, GPs, and allied health professionals
- Maintain accurate clinical documentation
Essential Criteria
- Current unrestricted AHPRA Registered Nurse registration
- Minimum 3 years' post-registration nursing experience
- Experience in community nursing, aged care, home care, disability, or chronic disease management
- Strong clinical assessment and critical thinking skills
- High standard of documentation and report writing
- Ability to work independently
- Strong communication and interpersonal skills
- Current driver's licence and reliable vehicle
- National Police Check
- Current CPR certificate
Desirable
- Experience completing clinical assessments and care planning
- Experience with Support at Home, Home Care Packages, CHSP, or NDIS participants
- Experience in wound care, chronic disease management, continence, or post-discharge care
