Southern Cross Care (SA, NT & VIC) Inc. is seeking a Home Care Coordinator to support person-centred services for clients within the Home Care portfolio. You will be the central contact for clients, carers and domestic staff, ensuring service plans and budgets are followed and issues escalated as needed.

The role requires solid interpersonal and time-management skills, and proficiency with computer systems including My Aged Care. A Diploma of Nursing with AHPRA is desirable but not required.
