The Eyre and Far North Local Health Network (EFNLHN) is seeking a committed Care Partner - Support at Home for the Lower Eyre region. The role is part time (0.6 FTE) and suitable for ASO3 Case Coordinators or Enrolled Nurses, with appointment and classification based on qualifications and experience.

Based in Port Lincoln, you will support older people to remain independent at home under CHSP and Aged Care frameworks, coordinating care plans and service access in a rural setting.
