Ability Street Community Services – Blacktown NSW is seeking an enthusiastic Care Coordinator with NDIS sector experience to lead our team of Support Workers and SIL homes across Sydney. You will ensure productive client management and delivery of person-centred supports in community settings.

Key duties include rostering staff to match client needs, completing paperwork, liaising with Participants, families and referrers, and reporting incidents per NDIS guidelines.
