BlueCare is seeking a Clinical Care Partner to oversee and coordinate complex clinical and care needs in community settings across Bowen, Proserpine and surrounds. The role involves assessment, care planning, managing budgets, liaising with multidisciplinary teams, and ensuring person-centred, high quality care.

You'll spend your day on the road, supporting clients in their homes, with Not-for-Profit salary packaging, travel reimbursement and discounts.
