In this rewarding role, you'll provide personalised in-home support that helps clients maintain their independence, wellbeing and quality of life while making a genuine difference in your local community.Your RoleAs a Care Assistant, you'll support older Australians and people living with disability to remain independent, safe and connected within their own homes and communities. Working closely with clients, families and the broader care team, you will deliver person-centred care and support in line with individual care plans.Your responsibilities will include:Providing high-quality care and support in accordance with personalised care plansSupporting clients to maintain their independence, wellbeing and quality of lifeAssisting with personal care tasks including showering, dressing, grooming and mobility supportAssisting with medication prompts and medication support in accordance with organisational policiesSupporting clients with domestic assistance, meal preparation, shopping and appointmentsMonitoring and reporting changes in clients' health, wellbeing or support needsMaintaining accurate documentation and client recordsWorking collaboratively as part of a multidisciplinary care team to deliver holistic client outcomesYour AttributesWe're looking for people who are passionate about making a genuine difference in the lives of others.
