As Team Leader, you will lead a small team of support workers. You’ll deliver hands-on personal care and community support while making sure care is protected, consistent and person-centred. You will be the main link between participants, families, the support team and our clinical and management staff.

What you’ll do
Lead, guide and mentor support workers on shift, including onboarding recent staff and buddy shifts
Provide personal care, daily living and community access support to female participants
Support medication administration and health monitoring in line with care plans (Enrolled Nurses: within your scope of practice)
Follow and help put into practice Behaviour Support Plans, risk assessments and care plans
Check shift notes, incident reports and documentation for accuracy and completeness
Report changes in participant health, behaviour or needs to the RN or management
Help with rosters, team meetings and day-to-day coordination of the house or service
Build positive relationships with participants, families, support coordinators and allied health

About you
Support Worker applicants:



Certificate III or IV in Individual Support, Disability or Mental Health (or similar), plus at least [2] years’ disability support experience, including some time in a lead or senior role
Enrolled Nurse applicants: current AHPRA registration as an Enrolled Nurse with no conditions, plus NDIS or disability experience
Experience with complex needs, mental health or behaviour support is highly regarded
Confident leader with robust communication, problem-solving and documentation skills
Calm, patient and respectful, with a genuine passion for supporting people with disability

NDIS Worker Orientation Module certificate
Full driver’s licence and a reliable, comprehensive-insured vehicle
Flexibility to work a mix of shifts, which may include weekends and sleepovers

Pay: $38.00 – $45.00 per hour

Expected hours: 15.0 – 40.0 per week
