St Vincent's Private Hospital Toowoomba is seeking a Registered Nurse or Enrolled Nurse to join as a Discharge Planner.
You will coordinate discharge planning, support secure and timely transitions, and engage with patients, families and community providers to arrange appropriate post-hospital care.
In this role you will work closely with clinical teams and Allied Health to remove barriers to discharge, maintain accurate records and ensure person-centred care throughout the patient journey.
