Cafs Therapeutic Residential Care in Ballarat, VIC, is seeking a Residential Care Case Manager to lead high-quality, therapeutic case management for young people where Cafs holds contracted responsibility.
You will coordinate care planning, therapeutic interventions and day-to-day residential care delivery, working with young people, families, residential teams, Child Protection, external case managers and partner agencies to support placement stability and secure, sustainable long-term care.
