Tradewind Australia is partnering with a respected not-for-profit aged care organisation to recruit an experienced Clinical Care Partner for a six-week locum assignment based in Hawthorn.
This is an immediate-start opportunity for an experienced clinician with extensive knowledge of the Support at Home (SAH) Program and a strong background in community aged care.
The successful candidate will play an important role in supporting older people to receive high-quality, person-centred care, ensuring their individual needs are assessed and appropriate services are coordinated to support their independence, wellbeing and quality of life.
Key Responsibilities
Conduct clinical assessments and develop individualised care plans.
Coordinate and monitor the delivery of safe, high-quality care and services.
Work collaboratively with GPs, allied health professionals and other service providers.
Monitor client needs and outcomes,



ensuring care plans remain relevant and responsive.
Maintain accurate documentation and ensure compliance with aged care and Support at Home requirements.
Provide guidance and support to clients, families and carers while promoting choice, independence and wellbeing.
Key Requirements
Bachelor of Nursing, Diploma of Enrolled Nursing or a relevant Allied Health qualification.
Extensive experience working within the Support at Home Program.
Solid background in community aged care, care coordination and clinical assessment.
Sound knowledge of aged care requirements, care planning and service delivery.
Excellent communication, organisation and stakeholder engagement skills.
Ability to work autonomously while contributing positively to a supportive team environment.
