Better Health Network is seeking a dedicated Young Person's Case Manager to provide intensive case management and outreach for *****-year-olds in Port Phillip, Victoria. Based at the Victorian Pride Centre, you will work across community settings and BHN locations to support housing stability and wellbeing.
As part of RhED, you will deliver trauma-informed, strengths-based support, collaborate with multidisciplinary teams, and advocate for access to housing, health, and education services.
