H1 Healthcare is seeking an experienced Care Coordinator to join our Sydney team. You will coordinate disability and in-home support for clients across NSW, with a focus on Icare-funded participants.

Expect a agile role balancing complex care plans, travel within NSW, and collaboration with families, case managers and allied health specialists. You will manage rostering, reporting, and advocacy to ensure clients access the right funding and services, while maintaining high safety and quality
