Wakefield Council is seeking a Senior Social Worker for the Mental Health Transitions Team (Ages *****) in Wakefield. This role supports young people as they move from children's services into adulthood within a team-oriented, relationship-based framework.
You will work with partners across CAMHS, Children's Services, SWYPT, education providers and ICB to create seamless pathways and advocate for recovery-focused approaches. Hybrid working and professional development are available.
