This role offers a combination of office-based work and community visits, with a focus on conducting home assessments and recommending modifications that support safe discharge and independent living.Key ResponsibilitiesConduct home and environmental assessmentsAssess clients' functional independence and support needsRecommend home modifications and assistive equipmentSupport secure discharge planning and transitions back into the communityComplete assessment reports and documentationLiaise with clients,
