Change Futures.As a Targeted Care Package (TCP) Support Worker, you'll play a vital role in supporting young people as they transition towards independence.
You'll provide practical, emotional and community-based support, helping young people build the life skills, confidence and resilience needed to thrive.
This role combines mentorship, advocacy, daily living support and community engagement to achieve meaningful outcomes for young people.What You'll Be DoingSupporting young people to develop independent living skillsAssisting with daily activities including cooking, cleaning and budgetingEncouraging participation in education, employment and community activitiesBuilding positive, professional relationships with young peoplePromoting healthy decision-making and positive behavioural outcomesAdvocating for young people's goals, wellbeing and rightsProviding a protected, therapeutic and trauma-informed environmentCompleting case notes, reports and administrative requirementsWorking collaboratively with case managers and multidisciplinary teamsSupporting young people to achieve their individual goals and aspirationsWhat We're Looking ForWe're seeking passionate, resilient and motivated people who genuinely want to make a positive impact in the lives of young people.You Will BringA positive, can-do attitude and strong problem-solving skillsThe ability to build trust and meaningful relationshipsExcellent communication and interpersonal skillsStrong teamwork and collaboration abilitiesCompassion, patience and emotional resilienceAn understanding of person-centred and trauma-informed practiceFlexibility to work across a rotating roster, including afternoons and weekendsA commitment to promoting safety,
