Grampians Health is seeking a Memory Support Nurse for Stawell Community Services on a fixed term, part time basis (0.4 FTE) until July ****.
The role focuses on dementia care, assessment and care planning, working with consumers, families and the broader health team to deliver person-centred, quality care.You will conduct memory assessments, develop care plans, provide dementia education and coordinate referrals to ensure integrated community services across the Grampians region.
