Your nursing experience can make a difference beyond the bedside.

Some of the most important moments in healthcare aren’t about treatment. They’re about listening, providing reassurance and helping someone feel supported when it matters most.

At Trilogy Care, we believe everyone deserves to approach the final stages of life with dignity, comfort and choice. We’re looking for an experienced Registered Nurse specialising in End of Life Care to help us make that possible.

As one of Australia’s fastest growing approved Support at Home providers, we’re bringing a fresh perspective to aged care, combining compassionate clinical practice with innovative technology and a genuine commitment to helping older Australians live well at home.

And here’s what makes this opportunity different.

You’ll use your clinical expertise to guide complex care, support families and empower our care teams, all from our modern Brisbane office. No bedside nursing, no rotating shifts and no weekends.

If you’re an experienced nurse who wants to continue making a meaningful difference while enjoying a more predictable work routine, this could be the career move you’ve been looking for.

The Role

As our End of Life Clinician, you’ll provide clinical oversight and coordinate personalised care for clients approaching the final stages of life. Working closely with clients, families, healthcare professionals and internal teams, you’ll help ensure care is compassionate, coordinated and aligned with each person’s wishes. You’ll also be a trusted clinical resource for our Team Leaders and Care Partners, providing guidance, mentoring and support through complex and sensitive situations.

This is an office based role,

with client interactions primarily conducted via phone and email.

What You’ll Do

- Conduct clinical and psychosocial assessments to understand clients’ changing needs
- Develop and review personalised End of Life Support Plans with clients and their families
- Coordinate access to palliative care services, specialists and multidisciplinary supports
- Provide clinical guidance and mentoring to Care Partners and Team Leaders
- Identify and manage clinical risks, escalations and complex care situations
- Support clients and families through sensitive conversations, care decisions and end of life planning
- Promote safe, compassionate and culturally respectful care in line with clinical standards and best practice

What You’ll Bring

- Current unrestricted AHPRA registration as a Registered Nurse
- At least two years of clinical nursing experience, ideally in aged care, palliative care or home care
- Confidence managing complex clinical needs and making sound clinical decisions
- Robust communication skills, empathy and emotional resilience
- Knowledge of end of life care, advance care planning and clinical risk management
- A collaborative approach and genuine passion for supporting clients and families through challenging times.

Experience in palliative care communication, grief support or clinical mentoring will be highly regarded.

Why You’ll Love Working at Trilogy Care

At Trilogy Care, you’ll have the opportunity to use your clinical knowledge, compassion and experience to support people through some of life’s most important moments.

Please note

To join our team, you’ll need to have full Australian work rights. As part of our standard recruitment process, we’ll also conduct a working rights check, police check and request professional references during onboarding.

We appreciate the interest of recruitment agencies but kindly ask that resumes not be submitted unless a prior agreement is in place. Trilogy Care will not accept responsibility for any fees related to unsolicited resumes.
